Duties and Responsibilities
1. Provide personal care, including baths, back rubs, oral hygiene, shampoos and change bed linens according to HHA plan of care (Patient care information in the plan of care).
2. Meets safety needs of patients and uses equipment safely and appropriately. This includes
maintaining a clean and orderly patient care area. Uses proper body mechanics.
3. Assists in dressing and undressing patients.
4. Assists patients at meal times. This includes and is not limited to feeding, meal preparation as
defined in the HHA plan of care/Daily Care information in the Plan of Care.
5. Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
6. Report on patient’s condition and significant changes to the supervising Nurse/manager/COO.
7. Assist in ambulation and exercise as directed in the HHA plan of care. (Daily care information in the Plan of Care).
8. To offer and assist with bedpans and urinals, colostomy care and catheter care.
9. Assist with personal laundry, light housekeeping related to patient care area.
10. Accurately and thoroughly document care provided on appropriate agency forms. Meet agency standards for clinical documentation.
11. Holds in strictest confidences all patient information and discloses information and data only to persons authorized by the agency
12. Responsible to learn and utilize technology (as an example but not limited to: phones, cell phones, computers) made available by agency to increase efficiency, communication, and performance of their duties.
13. Follows agency policies and procedures.
14. Participates in PI Plan as appropriate.
15. Participates in continuing education programs.
16. Other duties as assigned by supervisor.


Minimum Qualifications
1. Has successfully completed a home health aide training program or an equivalent training program that meets the specific requirements of the state in which the aide will work.
2. Has at least 1-year work experience in a home health agency, hospital, long term care facility or is presently enrolled in an accredited LPN/RN school of nursing with evidence of clinical skills
training.

3. At least 18 years of age.
4. Is a high school graduate or equivalent.
5. Is able to read, write, and carry out instructions.
6. Has physical ability necessary for extensive standing, lifting and pushing.
7. Demonstrates tact, patience and positive interpersonal skills necessary to work productively with
patients, families and co-workers.
8. Is flexible and cooperative in fulfilling role obligations.
9. Demonstrates good personal hygiene.
